The Functional Classification application “forms” are not required for changes within the Decennial Urban Area Boundary and Functional Classification process. Each form requires multiple signatures and would be too tedious for this process. They are only intended as an example of the type of information the Federal Highway Administration (FHWA) expects to see when requesting changes to functional classification
