All the forms on the page are designed for digital submission via e-mail.
Request for Commercial Inspection Form
The Request for Commercial Inspection form should be digitally completed and e-mailed to the SMO Commercial Inspection Group, E-Mail address: SM-StructuresCI@dot.state.fl.us at least 30 prior to the start of fabrication.
Welding Forms
The FDOT welding forms below should be digitally completed by the fabricator’s CWI, digitally signed, and e-mailed for review to the correct party, usually one of the SMO’s consultant firms assigned to the FDOT construction project.
These forms require no-cost digital signatures.
Fabricator QC/QA Plan Checklist Form
Each Production Facility that requests inclusion in the Structural Metals or Structural Coatings MAC Production Facility Listing is responsible to submit, in addition to other documents outlined in the FDOT Materials Manual, their current QC plan and a completed Fabricator QC/QA Plan Checklist (Form 675-070-17) to the SMO Steel/Coatings Group using the E-Mail address: SM-StructuresCI@dot.state.fl.us.
