External Guide (For Contractor's & Consultant CEI Staff)

This reference guide is for the purpose of preparing forms/documents for electronic submission to the Department. 
The process for submittal varies based on whether the form requires a signature.


Forms approved for electronic submittal can be viewed at the hyperlink below:



Section 1 - Forms That DO NOT Require a Signature:

1.    Download and complete the form/document.

2.    Email the form/document to the Appropriate Recipient.


Section 2 - Forms That DO Require a Signature:

1.    Download and complete the form.

2.    Print the form.

3.    Provide and/or obtain required Signatures on the form.

4.    Scan the form.

5.    Email the form as an attachment to the Appropriate Recipient.


Alternate Method -  If use of Digital Signatures has been approved by the Project Administrator:

1.    Download and complete the form/document.

2.    Attach authorized digital signature to the form in accordance with instructions from your Digital Certificate Provider.

3.    Email the form directly to the Appropriate Recipient.
